Inspector Notes
An initial subsidy health and safety inspection (SHSI) was initiated on 7/3/25 and completed on 7/22/25. The inspection started at 12:30pm and concluded at 2:30pm. There were 3 children present and 1 caregiver; the point total was 5. The inspector reviewed 3 children?s records and 2 caregiver/household member records on site on 7/2/25 and electronically on 7/22/25. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/30/25.
Standard 8VAC20-790-190-B
Children?s records shall include all of the required information. The records for child #1, child #2 and child #3 did not contain the following components: proof of child?s identity; address of the emergency contact; written statement that the vendor will notify the parent when the child is ill and the parent will arrange to have the child picked up as soon as possible.
The records for child #1 and child #3 did not include the work phone number of a parent.
Plan of Correction: The provider responded: Transitioning from the military program to city program. I will have parents fill out a second registration form to include all the proper requirements.
Standard 8VAC20-790-210-A
Each caregiver must be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). The documentation of a TB screening or test did not contain a statement that the provider was free of communicable TB.
Plan of Correction: The provider responded: A copy of the Tb results will be obtained on 8/5/25.

Areas and equipment inside and out shall be maintained in a safe and operable condition. There was a broken fence board in the backyard.
Plan of Correction: The provider responded: Fix board on fence.

Each bathroom area provided for children shall have sinks that are supplied with running warm water that does not exceed 120F. The water temperature of the bathroom sink used for washing hands registered as 133F when measured with a thermometer.
Plan of Correction: The provider responded: The temperature was adjusted to under 120F.
