Inspector Notes

An unannounced inspection was initiated on 09/23/2025 and completed on 11/17/2025 with local agencies, in response to a complaint received by the licensing office on 09/17/2025 relating to allegations of supervision, special care provisions, and emergencies. The inspector reviewed 2 children?s records and 2 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #1, who had been attending for over one month, did not have documentation of an immunization record on file.
Plan of Correction: Assistant Director will make sure that immunizations will be on file before child's first day of enrollment.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of Child #1, who had been enrolled for over 30 days.
Plan of Correction: Assistant director will make sure that students have a physical on file either prior to enrollment or within 30 days after enrolled.
Standard 8VAC20-780-570-C
The record of each child on formula shall contain the brand of formula and the child's feeding schedule.

Child #2 did not have a record on file of the brand of formula and the feeding schedule.
Plan of Correction: Staff will be sure to collect any information that include a child's name of formula and their feeding schedule before child's enrollment.
