Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
None of the records provided for the seven children in care today included documentation of a physical examination by or under the direction of a physician either prior to the child's attendance or within 30 days after the first day of attendance.
Plan of Correction: Provider response:
All parents were notified during the inspection that copies of their child's physical examination will be needed. In the future, newly enrolling parents will be notified that a copy of the child's physical exam must be provided within 30 days of attending.
Standard 8VAC20-800-60-B
A child's record shall contain the proof of the child's age and identity.
The records provided for child 4, 5, 6, and 7 did not include documentation that each child's proof of identity was viewed and by whom.
Plan of Correction: Provider response:
Proof of identification will be viewed and documented in each child's record within seven days of attending.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided in writing the provider's policies regarding the administration of medicine.
None of the 7 children's records reviewed included documentation of the providers decision and policies regarding the administration of medicine.

All parents will be provided and required to sign the provider's decision regarding the administration of medication form.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
The provider did not practice evacuation drills with the children in September, October, November or December 2025.
Plan of Correction: Provider response:
An evacuation drill will be practiced and documented for January 2026 and evacuation and shelter-in-place drills will be conducted and documented as required.
