Inspector Notes

An announced, on-site initial inspection was initiated and completed on July 17, 2025, as a part of the conditional licensure period. The on-site inspection began at 9:45am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were no children present and one caregiver; the point total was zero. The inspector reviewed no children?s records and two caregiver/household member(s) records on-site.
This inspection included document review , tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation.

The provider did not have a landline telephone available and operable at the time of the inspection.
Plan of Correction: The provider will contact the local cable provider to set up phone service in the family day home.
