During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site initial inspection was initiated and completed on 9/17/2025, as part of the conditional licensure period. The on-site inspection began at 12:00pm and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were no children present with the provider. The inspector reviewed 3 caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/25/2025. A POC submitted after this date will not appear on the public website.

A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate.

The family day home does not have a landline telephone.

The provider shall maintain a complete first aid kit with all required items.

The first aid kit was missing at least two triangular bandages.

The provider shall ensure emergency telephone numbers, including 911 or local number for police and fire, emergency backup provider contact information, and the regional poison control center, are posted in a visible area close to the telephone.
