During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An announced, on-site initial inspection was initiated on and completed on 8/5/2025. The on-site inspection began at 9:10am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were no children present and one caregivers. The inspector reviewed four caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-330-A
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate. The landline at the home required electricity.
Plan of Correction: The provider will purchase a landline telephone that does not require electricity.
Standard 8VAC20-800-430-B
Accordion expansion gates and pressure mounted gates shall not be used as protective barriers at stair openings. There was a pressure mounted gate used as a protective barrier at the stair openings from the basement up to the ground floor.
Plan of Correction: The provider will install a gate at the bottom of the basement stairs that mounts to the wall.
Standard 8VAC20-800-760-A-1
A first aid kit must contain triangular bandages, and a digital thermometer. The provider's first aid kit did not contact triangular bandages or a digital thermometer.
Plan of Correction: The provider will purchase triangular bandages and a digital thermometer for the first aid kit.
Standard 8VAC20-800-770
A working battery-operated flashlight, a working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times. The radio did not have batteries during the inspection.
Plan of Correction: The provider will purchase batteries for their radio.
Standard 8VAC20-800-800-A
The family day home shall have a written emergency preparedness and response
plan that: includes emergency evacuation, emergency relocation, and shelter-in-place procedures; addresses the most likely to occur scenarios, including but not limited to fire, severe storms, flooding, tornadoes, and loss of utilities; and includes provisions for a responsible person who is 18 years of age or older and is able to arrive at the family day home within 10 minutes for emergency backup care until the children can be picked up by their parents. The provider did not have a written emergency preparedness and response plan.
Plan of Correction: The provider will complete an emergency preparedness and emergency response plan.
Standard 8VAC20-800-940-B
The provider shall have an operable thermometer available to monitor refrigerator and freezer compartment temperatures. There were no thermometers in the refrigerator or freezer.
Plan of Correction: The provider will purchase a thermometer for their refrigerator and freezer.
