REPEAT VIOLATION
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff A, employed for over two months, did not have documentation of requesting a central registry check from one state, and no documentation of requesting a central registry check from a different state.
Staff C, employed for over four months, did not have documentation of requesting a central registry check from one state and no documentation of an out of state sex offender search.

Plan of Correction: We have worked with Senior Leadership and HR / On-boarding Team to ensure no staff are hired without proof of submission of out of state background checks. Staff A's and Staff C's central registry check and out of state background checks have been completed.

Plan of Correction: Child #2's file is complete with immunizations. We are working on a process to ensure immunizations are received prior to enrollment which may include a release of information to obtain immunizations directly from the school

Plan of Correction: Family submitted child's physical examination online on 11.7.25. We are working on a process to work with families and the district to ensure we obtain a physical exam within 30 days of attendance.

The center is required to obtain documentation of follow-up tuberculosis (TB) screening or tests at least every two years from the date of the initial screening or testing.
Staff B's most recent tuberculosis (TB) screening was expired for over one month.

Plan of Correction: Staff B received TB screening on 11.3.25. Area Manager has created a spreadsheet in order to minotor two year expirtion dates and also has them entered on a calendar 23 months out.
