The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #1, who had been in care for over 30 days.
Plan of Correction: Parent informed of incorrect document; document has been requested to be provided within 2 weeks. Provider has printed out copies of blank form to provide to future enrollees as part of their enrollment packet to prevent this from reoccurring.
