Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. Child 4 and Child 5, enrolled for over 2 months, did not have the model form "Child's Registration Form" or an equivalent. The children were missing the child and parent's home address, the parents employment information, the child's doctor's name and phone number, two emergency contacts, allergy and health information, and written agreements.
