The center is required to do the appropriate background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from two previously resided states for Staff 1, who has been employed for more than a month and was working with children at the time of the
inspection.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child 1, Child 4, and Child 5, enrolled for 2 months, did not have documentation of an immunization record on file.

Documentation of a physical examination must be submitted for each child within 30 days after the first day of attendance. Child 31, Child 4, and Child 5, enrolled for over 2 months, did not have documentation of a physical.

There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. The two staff on site did not have documentation of daily health observation training. The licensee confirmed that the staff do not have daily health observation training.

Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. The children did not wash hands after eating afternoon snack.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. Child 4 and Child 5, enrolled for over 2 months, did not have the model form "Child's Registration Form" or an equivalent. The children were missing the child and parent's home address, the parents employment information, the child's doctor's name and phone number, two emergency contacts, allergy and health information, and written agreements.
