Repeat violation - The center is required to do the appropriate background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one previously resided state for Staff 1, who has been employed for 3 months and was working with children at the time of the inspection.

The current form required by the Virginia Department of Health or a physician's form shall be used to report immunizations received and the results of the required physical examination. The physical on file for Child 1 was not a physician's form or the VDH form.

The center shall have a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and relocation, shelter-in-place, and lockdown. The emergency preparedness plan and the posted emergency maps designate the kitchen off the cafeteria as the space to meet when the children need to shelter in place. The kitchen was locked during the inspection on 2/09/2026. Staff did not have a key to the kitchen, and did not have a plan for an alternate place to shelter in place.
