During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site 60-day focused inspection was initiated and completed on 09/11/2025, as part of the licensure period.

The on-site inspection began at 9:00 am and ended at 10:15 am. The inspector reviewed compliance in the areas listed above.

There were 13 children present and 6 staff. The inspector reviewed 3 children?s records and 3 staff records on-site.
This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 09/23/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
A list was not available for review in the infant room.
Plan of Correction: The allergy list has been put up in the room and covered for confidentiality.
Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill.

An evacuation drill for the month of August 2025 was not completed.
Plan of Correction: Have one planned for this week.
Standard 8VAC20-780-560-G
When food is brought from home, the food container shall be clearly dated and labeled in a way that identifies the owner.
In the Infant room, the following was not labeled or dated as required:
* For Child #4 - one baby food jar
* For Child #5 - one applesauce pouch, one bento box, one sippy cup with milk
* For Child #6 - one applesauce pouch
Plan of Correction: Message to parents was sent reminding to label. We have printed labels for teachers in case its missing.
