When staff are supervising children, they shall always ensure their care, protection, and guidance.
On Friday, January 16, 2026, Child #1 (age 3) was throwing large plastic blocks around the classroom. Staff #1 picked up a block and threw it in the direction of Child #1. Staff #1 walked to the child, then took Child #1 by the arm and moved the child onto a mat for a timeout. Staff #1 was heard stating in a raised voice, ?Sit down here and don?t get up.? Staff #1 did not redirect the behavior appropriately and did not ensure care, protection, and guidance.

Staff #1, who did not meet lead teacher qualifications, was alone with a group of preschool children for approximately fifteen minutes while the lead teacher was on break.
