REPEAT VIOLATION

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

At the time of the inspection, Staff #5, who had been employed for five months, did not have documentation of requesting a sex offender registry from one state.

At the time of the inspection, Staff #3, who had been employed for three months, did not have documentation of requesting a criminal history check from one state.
Plan of Correction: The sex offender registry was pulled and placed on file the same day for Staff #5. ? The out-of-state criminal background check (Pennsylvania) for Staff #3 was completed, cleared, and filed on the same day as the inspection.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

At the time of the inspection, Staff #2 did not have documentation of completing the course, which is overdue by two months.

The required first aid kits shall include at a minimum: triangular bandages.

At the time of the inspection, the facility?s first aid kit was missing triangular bandages.
Plan of Correction: A triangle bandage was ordered, received, and placed in the first aid kit.

Any vehicle used by the center for the transportation of children shall meet the following requirements: The vehicle shall be insured with at least the minimum limits established by
Virginia state statutes.

At the time of inspection, the insurance for Vehicle #2 had expired by 20 days.
Plan of Correction: Updated insurance cards were placed in all buses on the same day.
Standard 8VAC20-780-60-A
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #6 has documented by a physician noting a food allergy on their physical examination form; however, there is no written care plan to include the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Plan of Correction: The allergy care plan for Child #6 has been initiated, and we are in the process of updating all records with families. An email has already been sent to parents.

Documentation demonstrating the individual has completed orientation training as required in 8VAC20-780-240 and 8VAC20-780-245.

At the time of the inspection, Staff #8, who had been employed for five months, did not have documentation demonstrating the individual has completed orientation training.
