Inspector Notes

An announced, on-site initial inspection was completed on 9/10/2025, as a result of a planned change in ownership. Any areas of noncompliance are documented in the violation report. The on-site inspection began at 8:00 am and ended at 9:00 am. The inspector reviewed compliance in the areas listed above.

This inspection included: document review, electronically, and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
Chipped paint was found in the following areas:
Room #2 chipped paint on the window sill located near the window acing the parking lot.
Room #6 chipped paint on the wall located under the window that faces the outside AC units.
Plan of Correction: Touch up paint within 30 days, monitor area for chipping and remove chipped paint.
