Diapering Procedures were not followed as required.
1. On January 13, 2026 staff #2 changed the diaper of a child on the floor in the two-year-old room. For children younger than three years, diapers shall be changed on a changing table or countertop designed for changing.
2. On January 13, 2026, three children did not have any reported diaper changes recorded on the parent communication software.
