The center must request a search of the central registry prior to the employee's first day of employment and by the end of the 30th day of employment.

1.Staff #1, who has been employed for more than two months, had a central registry search requested 8 weeks after their hire date.

2.Staff #2, who had been employed for more than three months, had a central registry search requested more than 8 weeks after their hire date.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of Child #4, who had been enrolled for more than two months.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #2 had no documentation and has been employed for more than three months.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: Date and time of injury, name of injured child, type and circumstance of the injury, staff present and treatment, date and time when parents were notified, any future action to prevent recurrence of the injury, staff and parent signatures or two staff signatures, and documentation on how parent was notified.

1. Injury Report #1 did not have the following information : date and time of when parents were notified, no signatures from staff or parents.
2. Injury Report #2 did not have the following information: date and time of when parents were notified, no second signature of either an additional staff or parent.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

1. The record for Child #1 and Child #3, enrolled for more than two months, was missing the following required components: Documentation of viewing proof of the child's identity and age.

2. The record for Child #2, enrolled for approximately six weeks, was missing the following required components: name, address, and phone number of a second emergency contact and previous child day care and schools attended by the child.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.

1. Staff #1, who has been actively working at the center for more than two months, did not have a second completed reference verification on file.

2. Staff #2, who has been actively working at the center for more than three months, did not have a second completed reference verification on file.
