An announced, on-site initial inspection was initiated on and completed on 9/3/2025, as a part of conditional licensure period. The on-site inspection began at 11:05am and ended at 2:15pm. The inspector reviewed compliance in the areas listed above. CDC: There were not any children or staff present as this was an initial inspection. 3 staff records were reviewed on site. All areas of standards were reviewed. However, areas that were not able to be determined will be reviewed at the first unannounced inspection. This inspection included document review, tour of the facility, interviews, observations (this would include observations, and measurements.
