A sworn statement or affirmation disclosure was not available for review for Household Member (HHM) #3. This was confirmed through the Fairfax Office for Children (OFC).

Plan of Correction: We took a visit to OFC to submit this paperwork for HHM #3. It will to on file when obtained. OFC stated to LI that their office will send an updated report to OCCHS.
