During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 03/27/2026. This inspection was conducted due to the recent move to this new address. The inspection started at 10 am and concluded at 11 am. There were (2) children present with (1) resident child, and (1) caregiver; the point total was (8). The inspector reviewed (3) children?s records and (2) caregiver/household member records on-site on 03/27/2026.

This inspection included: document review (i.e. Files, policies and procedures, emergency drill logs), tour of the facility, interviews, and observations. Information gathered during the inspection determined that (2) applicable standards were in non-compliance. These violations were documented on the violation notice issued to the home.

Standard 8VAC20-790-190-B
The child's information shall include the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.

The parent of Child #1 had completed this section with "N/a." Child #1 has been enrolled for 5 months.
Plan of Correction: The parents of Child #1 will be contacted to find a person to act as an emergency contact to be listed.
Standard 8VAC20-790-420-A
The vendor shall have a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown, and shelter-in-place procedures.

The vendor did not have a written emergency preparedness plan completed for the new address.
Plan of Correction: The LI will send a blank EPP to be completed by the vendor for her new address.
