Inspector Notes
An initial inspection was initiated and completed on July 24, 2025, as a part of the licensure period. The on-site inspection began at 11:30 a.m. and ended at 12:30 p.m. The inspector reviewed compliance in the areas listed above. Two staff records were reviewed. This inspection included document review tour of the facility, and measurements.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
