Inspector Notes

An announced, on-site initial inspection was initiated on 8/13/25 and completed on 8/18/2025, as a part of licensing process. The on-site inspection began at 9:30 AM and ended at 10:45 AM. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 caregiver; the point total was 2. The inspector reviewed 2 children?s records and 4 caregiver/household members records on-site.
This inspection included: document review, tour of the facility (additional space inspected 8/18/25), interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation.

A child's record shall contain the following information: parent's name, address, phone number, parent's work information, first day of attendance, 2 emergency contacts, authorization for emergency care, Insurance policy name and number.
