During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 10/16/2025, as a part of the conditional licensure period. The on-site inspection began at 12:25 PM and ended at (1:25 PM. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 8. The inspector reviewed 2 children?s records.
This inspection included: document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/28/2025. A POC submitted after this date will not appear on the public website.

Child #2 's immunization record was not signed by a licensed physician, physician's designee, or an official of the local health department.on file. The report for immunizations was overdue by 3 weeks.
A child's record shall contain the following information: parent's name, address, phone
number, parent's work information, first day of attendance, 2 emergency contacts information,
authorization for emergency care, medical insurance policy name and number.
Child #1's record was missing: emergency contact 's phone number. Information was overdue by 2 months.
Child #2's record was missing: Parent information, parent work information, Doctor's information, any allergy information, medical insurance name and policy number, authorization for emergency care. Information was overdue by 3 weeks.
Before the child's first day of attendance the provider shall obtain the parent's written
acknowledgment of the receipt of the information regarding family day home operations and administration.
Missing acknowledgments for Child #2: Signed acknowledgement of all policies and
procedures, Acknowledgment of the decision to administer medication and Acknowledgment
of the declaration of liability insurance. Information was overdue by 3 weeks.
A first aid kit that contains at a minimum: 2 triangular bandages and a digital thermometer,.
The first aid kit was incomplete. Missing items included: a second triangular bandage nad a digital thermometer.
The family day home shall have a written emergency preparedness and response plan.
There was no written emergency preparedness plan available for review.
There was no snack menu dated or posted.
