Repeat:
Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers shall be stored in areas inaccessible to children.

Items labeled "keep out of reach of children" were accessible to children ages 17 months to 2 years of age.
Plan of Correction: The toothpaste was removed immediately and relocated.
Standard 8VAC20-800-530-I
Mattresses, when used, shall be covered with a waterproof material that can be cleaned and sanitized.
Two mattresses were not covered with a waterproof material to enable them to be cleaned and sanitized properly.
Plan of Correction: I will put the original mattresses back in both cribs.
