Inspector Notes

An unannounced, on-site 60 day inspection was initiated on 10/23/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 11:40 and ended at 12:55. The inspector reviewed compliance in the areas listed above. There were 33 children present and 10 staff. The inspector reviewed 0 children?s records and 1 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The most recent licensing paperwork is not posted.
Plan of Correction: We will make sure we post the most recent licensing paperwork.
Standard 8VAC20-770-60-B
An employee or volunteer of a licensed or registered child day program or of a family day home approved by a family day system must not be employed or provide volunteer service until the agency or home has the person's completed sworn statement or affirmation.

A staff ,Staff 1, at the center in a classroom is missing the sworn disclosure documentation.
Plan of Correction: We had her fill this out during the inspection.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The center has a new owner and the children's files do not have the statement that the files can be used by the new owner.
Plan of Correction: We will make sure all the parents will sign the form by Monday.
Standard 8VAC20-780-70
A staff record that includes information regarding references and emergency contacts as well as other information shall be kept at the center.

A staff member, Staff 1, is missing the emergency contact and the two references.
Plan of Correction: We will get this done as soon as possible.
