An announced, on-site initial inspection was initiated and completed on September 8, 2025. The on-site inspection began at 9:10am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were no children present and 2 staff. The inspector reviewed 2 staff records on-site. This inspection included a tour of the facility, document review, observation, and interviews.
