The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff #2, employed for more than 4 months, did not have documentation of requesting a central registry check from one state; and Staff #3, employed for approximately 1 month, did not have documentation of requesting a central registry check from a different state.

Plan of Correction: A request will be sent out for the state in regard to Staff #2; and a request will be sent out for the state in regard to Staff #3. Both requests will be sent within 10 days. Once the findings are received, each will be shared and reviewed with the Licensing Inspector. Until such time, both staff will be supervised (within sight and sound) by the Center Director.
