Each physical report shall include the date of the physical examination. The physical exam in the record for Child #2 did not contain the date of the physical exam.
Plan of Correction: Provider will request more documentation of the child's physical that indicates the date the physical was completed.
Standard 8VAC20-800-70-B
The provider is required to obtain the parent's written acknowledgement of the receipt of all required information. The records for Child #1 and 2, who have been attending the family day home for two weeks, did not contain written acknowledgement of the receipt of the provisions of the emergency plan.
Plan of Correction: Provider will obtain the signatures from the parents.

The provider shall document in the child's record the method of verification of the child's age and identity. The provider did not document the date the proof of identity documents were viewed for Child #1 and 2 who have been attending the family day home for two weeks.
