Standard 8VAC20-780-240-B
Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Staff 1 did not have documentation of orientation until one month after assuming job responsibilities.

Medication shall be given to a child only with written authorization from the parent. One
emergency medication for Child A did not have written authorization from the parent.

Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. A medication was in a unlabeled Ziploc bag without the child's name, dosage amount, or times to be given.

Repeat violation - Each center shall keep a separate record for each child enrolled that contains all of the required information. Child 4 did not have the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Child 6 only had one of two emergency contacts documented.
