During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site initial inspection was initiated and completed on 10/28/2025. The on-site inspection began at 1:25 PM and ended at 2:30 PM. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregivers; the point total was 6. The inspector reviewed 2 children?s records and 3 caregiver/household members records. This inspection included document review, a tour of the family day home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The applicant and adult household members shall obtain the results of a search of the central registry.

The applicant and household member #1's records do not contain the results of a central registry search.
Plan of Correction: The request was submitted but results were not received. I will contact OBI today to get the results.

A person living in an applicants home between 14 and 18 years of age must have a child protective services central registry check.

Household member #2's (15 years of age) record does not contain the results of a central registry check.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Child #1's record does not contain a physical exam. Child #1 has been enrolled at the family day home for approximately 7 weeks.
Plan of Correction: I will ask the family to get the physical to me by next week.
