During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on October 27, 2025. The on-site inspection began at 9:30 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were eight children present and three staff. The inspector reviewed five children?s records and five staff records. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 7, 2025. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #2, observed working with children, did not have documentation of requesting a central registry check and sex offender search from one state.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #3, who had been enrolled for seven weeks.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. The record for Staff #2, observed working with children, did not have a TB screening.

Inside areas occupied by children shall be maintained no lower than 68 degrees F. At approximately 10:05 am, the temperature in the building was 65 degrees.

The record for Staff #1, employed for a month, did not have documentation of the qualifications for the position. The record for Staff #2, present working on this date, did not have documentation of two reference checks and orientation training. The record for Staff #5 did not have documentation of qualifications for the position and two reference checks.
