Hazardous substances must be in a locked place that prevents access by children. In the toddler classroom, bleach/water solution and cleaning solution were stored on a open shelf in the bathroom, out of reach of children.
Plan of Correction: A lock box will be added to the room.

The sponsor shall maintain public liability insurance for bodily injury with a minimum limit of at least $500,000 each occurrence and with a minimum limit of $500,000 aggregate. The liability insurance coverage expired 11/22/2025.
Plan of Correction: Insurance was secured effective 1/13/2026.
Standard 8VAC20-780-520-C
Diaper ointment is to be labeled with the child's name and stored inaccessible to children. In the infant room, diaper ointment was stored on a open shelf under the diapering changing table within reach of children. The ointment was not labeled with the child's name.
Plan of Correction: The diaper cream was labeled with the child's name immediately and placed out of reach of children. Director will meet with all staff to remind them of the requirements for diaper ointment.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. At approximately 10:45 a.m., there were 5 children present in the infant room, but only 4 were marked present on the attendance record.
Plan of Correction: Staff will be reminded to slow down and note attendance in the correct room when a child moves to a different room. The correct attendance was noted in the app immediately.
