No soft bedding of any kind shall be used under or around infants.

A child's record shall contain the following information: name, office address and telephone number of the child's physician; name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; name and policy number of the child's medical insurance, if applicable; and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.

Before the child's first day of attendance, parents shall be provided in writing the following information: check in and check out procedures; whether or not there is liability insurance; requirement for the child to be adequately immunized; requirement for paid caregivers to report suspected child abuse or neglect; custodial parent's right to be admitted to the family day home any time the child is in care; general daily schedule; policies for the provision of food; discipline policies; amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child; address of the website of the department.

Parents of Child #1, #2, #3, and #4 did not receive check in and check out procedures; whether or not there is liability insurance; requirement for the child to be adequately immunized; requirement for paid caregivers to report suspected child abuse or neglect; custodial parent's right to be admitted to the family day home any time the child is in care; general daily schedule; policies for the provision of food; discipline policies; amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child; address of the website of the department before the child's first day of attendance. Child #1, #2, #3 and #4 have been attending the family day home for over two months.

Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent verification of the identity and age of the child.
