Inspector Notes

An announced, on-site initial inspection was completed on 9/23/2025, as a part of the licensure process. The on-site inspection began at 12:08 pm and ended at 1:50 pm. The inspector reviewed compliance in the areas listed above. There were no children present. The inspector reviewed 1 staff record on-site.

The inspection included a document review, measurements, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Agent #1, who is empowered to act on behalf of the corporation in matters relating to the facility, did not have a completed child abuse and neglect search for a residence outside of Virginia in the past 5 years.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #1?s tuberculosis screening was dated seven months prior to date of employment.

Emergency evacuation and shelter-in-place procedures or maps shall be posted in a location conspicuous to staff and children on each floor of each building.
