Inspector Notes

An announced initial inspection was completed on October 21, 2025 as a result of a planned change in ownership. The on-site inspection began at 9:45am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 22 children present and seven staff. The inspector reviewed five children?s records and eight staff records on-site. This inspection included document review , tour of the facility, interviews, and observations.

Any areas of noncompliance are documented in the violation notice.

Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member, is considered part of the staff member's records, and shall include required information.

The record for Staff #6 did not contain documentation of staff orientation. Staff #6 has been employed for one month.
Plan of Correction: Staff #6 will complete staff orientation by 10/24/2025. Moving forward, management will ensure to document staff orientation in staff member's records.
