Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on April 2, 2026 as a part of the conditional licensure period. The on-site inspection began at 9:40am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 36 children present and eight staff. The inspector reviewed five children?s records and six staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
2Child #6 has a daily medication on site. The medication has been administered for a total of 10 days, 03/19/2026-04/01/2026. There is no record of the administration of Medication 1. Staff #4 confirmed that there was not a record kept of the medication given to Child #6.
Staff records shall be kept for each staff person.
Staff #1's record, did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position. Staff #1 has been employed for one month.
