A subsidy health and safety inspection (SHSI) was initiated on August 11, 2025 and completed on August 11, 2025. The inspection started at 1:30 p.m. and concluded at 2:30 p.m. The inspector reviewed two caregiver/household member records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
