An unannounced, on-site Initial Inspection was conducted on October 15, 2025. The on-site inspection began at 3:00 pm and ended at 6:00 pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed the office and agent /agent record on-site on 10/15/2025. Additional records were reviewed electronically on October 28, 2025. This inspection included document review, tour of the facility, interviews, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
