A subsidy health and safety inspection (SHSI) was conducted in conjunction with the initial inspection. Subsidy standards that are not covered in voluntary registration standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The child?s information shall include all required information.

Child #1 was missing proof of child?s identity, written authorization for emergency medical care, permission to transport child, a written statement that the vendor will notify the parent when the child becomes ill.

Standard 8VAC20-790-230-A
Before a child may attend the family day home, the vendor shall obtain documentation that the child has been immunized.

Child #1 had an immunization dated five days after first day of attendance.

All caregivers shall have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Staff #1 had a CPR certification that expired six months ago.
