Inspector Notes

An announced, on-site Initial inspection was initiated on 9/25/2025 and completed on 9/25/2025. The on-site inspection began at 9:10AM and ended at 11:15AM. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 caregivers; the point total was 10. The inspector reviewed 4 children?s records and 2 caregiver/household member(s) records on-site.
This inspection included document review, a tour of the facility, interviews and observations.

Standard 8VAC20-800-320-B
Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

4 outlet covers were missing, out of 24 outlets. These were missing in areas where children were allowed to play.
Plan of Correction: Outlets covers will be used at all times during hours of operation. [The outlet covers were placed while the licensing inspector was on site.]
Standard 8VAC20-800-330-A
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate. Cordless or cell phones may be used in addition to the landline telephone.

The Provider had a cordless phone, but did not have a landline as required.
Plan of Correction: A landline will be purchased right away.
Standard 8VAC20-800-60-B
A child's record shall contain the following information: Documentation of the review of the child's emergency contact information as required by 8VAC20-800-780.B. Also, written authorization for emergency medical care.
The record for Child #1 did not have the annual update. The last date the document was reviewed was 7/10/2024. The record for Child #2 did not have the emergency medical authorization signed by the parents.
Plan of Correction: The documents will be updated/signed by parents by 9/29/2025.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided in writing the following information: Policies for the administration of medications.
The Decision to Administer Medications form was not given to 4 out of 4 students enrolled.
