An unannounced, on-site focused inspection was conducted on 12/11/25 from 10:25 am to 10:50 am, as a part of the conditional licensure period. The inspector reviewed compliance
in the areas listed above. There were no children present . The inspector reviewed 2
children?s records and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
