3Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #2 has attended the center for three days and does not have an immunization record on file. Child #4 was at the center for three weeks before the immunization records were obtained.
Plan of Correction: Immunization will be obtained for children observed (files) and upon date of enrollment moving forward.
Standard 8VAC20-780-150-B
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. The immunization and physical record for child #5, in attendance for six months, were not signed by a physician, his designee, or an official of a local health department.
Plan of Correction: Ensure all pertinent info needed to complete file is correct.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence that include a staff and parent signature or two staff signatures. An injury report dated 2.9.26 for child #6, only contained the parent signature.
Plan of Correction: Lead teacher will have all accidents/incidents signed by director.
