Inspector Notes

An initial subsidy health and safety inspection (SHSI) was initiated and conducted on 9/4.2025. The on-site inspection began at 7:20 AM and ended at 8:34 AM. There were seven children in care with two staff. A sampling of five children's records on site. Two staff records were reviewed via email on 09/08/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/15/2025. A POC submitted after this date will not appear on the public website.

During the 90-day period, there must always be at least one staff with current CPR certification present during operating hours of the center.
Staff 2 has CPR certification however it was not obtained from an entity that requires an in-person competency demonstration.

Standard 8VAC20-790-640-A
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
There were two containers of hand sanitizer on shelving units and a container of white correction fluid in an unlocked staff desk, all of which had warning labels.

Standard 8VAC20-790-790-A
The vendor shall have a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures. The plan shall address the most likely to occur emergency scenarios, including severe storms, loss of utilities, natural disaster, chemical spills, intruder, and violence on or near the facility, and facility damage or other situations that may require evacuation, lockdown, or shelter-in-place.
