Inspector Notes

An unannounced, on-site initial inspection was initiated and completed on 10/6/2025. The on-site inspection began at 10:30 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, interviews, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/14/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus. There was no resilient material provided for the play equipment which had a climbing apparatus.
Plan of Correction: Will have mulch placed in the area underneath playset area.
Standard 8VAC20-780-380-A
There shall be a posted daily schedule. There was no schedule posted in the Toddler room or Pre-K room.
Plan of Correction: Hung up on 10/6/2025.
Standard 8VAC20-780-540-C
The required first aid kits shall include all required items. There was no thermometer in the first aid kit or facility.
