During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 3/25/2026, as a part of the conditional licensure period. The on-site inspection began at 1:15am and ended at 2:35pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregivers; the point total was 16. The inspector reviewed 4 children?s records and 3 caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/14/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in safe condition. There is a top loading freezer that does not have child proof latch. There are cubbies attached to walls with child proof latches and not secure anchors to prevent toppling.
Plan of Correction: Provider has installed a child-proof latch on top loading freezer to prevent access. All cubbies have been secured to the wall using proper anchor to prevent tipping.
Standard 8VAC20-800-320-A
Small electrical appliances such as, but not limited to, curling irons, toasters, blenders, can openers, and irons shall be unplugged unless being used by the caregiver or with children under close supervision. There was a bottle warmer plugged into an outlet but was not in use.
Plan of Correction: Provider has unplugged the
bottle warmer and will ensure
that all small electrical
appliances are unplugged
immediately after use.
Standard 8VAC20-800-450-A
Doors with clear glass panels that reach within 18 inches of the floor shall be clearly marked with decorative objects such as pictures, artwork, or decals at the eye level of children in care. The sliding glass doors do not have any decorative objects at eye level of children in care.

Standard 8VAC20-800-490
The climbing portions of indoor slides and climbing equipment over 18 inches high shall not be over bare floor. A climb and slide measuring 20inches high was located over a bare floor.

Standard 8VAC20-800-520-A
Cribs shall be provided for children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat, or bed. There was not a crib set up and available for one infant in care under the age of 12 months.

Standard 8VAC20-800-560-A
A caregiver shall be physically present on site and provide direct care and supervision of each child at all times. Direct care and supervision of each child includes:1. Awareness of and responsibility for each child in care, including being near enough to intervene if needed; and 2. Monitoring of each sleeping infant in one of the following ways: a. By placing each infant for sleep in a location where the infant is within sight and hearing of a caregiver. A six month old was left unattended on the couch when the provider went to answer the front door.
Plan of Correction: Moving forward, provider
will ensure direct
supervision of all children
at all times. When needing
to answer the door or step
away, the infant will be
placed in a safe crib play
pen and/or brought along
to maintain sight and
hearing supervision.
