During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 0/00/0000 , as a part of the licensure period. The on-site inspection began at 9:30am and ended at 12pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-245-M
Documentation of training shall be kept by the center in a manner that allows for identification by individual staff member, is considered part of the staff member's record, and shall include
the number of training hours and evidence that training on each topic required in this section has been completed.
Documentation of required training was not kept in a manner that includes the hours of training and required topics for Staff #1 and #2. Staff #1 has been employed for approximately 3 months and Staff #2 has been employed for approximately 6 weeks.
Plan of Correction: Both staff have received the required training. The course certificates will be printed and dated and the topics clearly documented and maintained in each staff file.
Standard 8VAC20-780-500-B
The diapering area shall have a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

The diaper area in Classroom 2 and 3 did not have a diapering disposal system that did not require the staff member's hand to touch the exterior of the surface of the system.
Plan of Correction: Two new diaper disposal systems will be purchased for both these classrooms.

At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care.
Staff #1 and #2 were the only staff in Class 1 and Class 2. Neither staff had certification in CPR.
Plan of Correction: Both staff are enrolled in the CPR/First Aid course scheduled for 4/1/2026.

At least one staff in each classroom or area where children are present shall have current certification in first aid.

Staff #1 and #2 were the only staff in Class 1 and Class 2. Neither staff had certification in first aid.
