When staff are supervising children, they shall always ensure their care, protection, and guidance.

On December 24, 2025, Child#1 (age four years old) left the center without the staff knowledge for approximately 20 minutes. Child#1 traveled approximately 0.2 miles on the roadway before being found by unknown person and transported to local business where child was reunited with caregiver. Staff #1 left Child #1 unattended to take a restroom break. Upon returning, Staff #1 discovered that Child #1?s whereabouts were unknown and that the entrance door was opened.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On December 24, 2025, Child#1 (age four years old) was without actual sight and sound supervision of staff for approximately 20 minutes when the child left the center and was found unaccompanied alongside the roadway approximately 0.2 miles from the center. The child was transported by an unknown person to a local business until local agency representative arrival.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The center?s Child Safety and Supervision Policy states that children must always be supervised and that no child shall ever be left alone. On December 24, 2025, the center failed to follow its own policy when Staff #1 left Child #1 unattended to take a restroom break. Upon returning, Staff #1 discovered that Child #1?s whereabouts were unknown and that the entrance door was opened.
