During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An announced, on-site initial inspection was initiated and completed on 10/17/2025 , as a part of the licensure period. The on-site inspection began at 11:00am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 105 children present and 26 staff. The inspector reviewed 10 children?s records and 6 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/28/2025. A POC submitted after this date will not appear on the public website.

No staff members, including bus drivers who care for children with emergency medications, are MAT certified.
-Child #1's record (enrolled over 3 years ) was missing work phone number and place of employment of each parent and addresses for two emergency contacts.
- Child #2's record (enrolled for 2 months) was missing work phone number and place of employment of each parent.
-Child #4's record (enrolled for over 7 months) was missing a second emergency contact, including a name, address, and phone number other than the parent's
-Child #6's record (enrolled for over three years) was missing emergency contact addresses, work phone numbers, and places of employment for each parent.
-Child #8's record (enrolled over 1 year) was missing two emergency contact's addresses other than the parents.
-Child #10's record (enrolled over 1 year) was missing an annual update of information in child's record.
