The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

There was no sworn statement for Staff #1 and #3, who were working during this inspection.

Each child day program that is licensed pursuant to this chapter and any program described in subdivision A 4, B 1, or B 5 of ? 22.1-289.030 that serves preschool-age children shall develop and implement a plan to test potable water from sources identified by the U.S. Environmental Protection Agency as high priority, including bubbler-style and cooler-style drinking fountains, kitchen taps, classroom combination sinks and drinking fountains, home economics room sinks, teacher's lounge sinks, nurse's office sinks, classroom sinks in special education classrooms, and sinks known to be or visibly used for consumption.

Lead testing has not been implemented at this center.

Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening.

There was no documentation of a negative tuberculosis screening for staff #3.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

None of the staff had documentation of completing the Virginia Department of Education-sponsored orientation course.

There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

There were no staff members with Daily Health Observation training.

Staff records shall be kept for each staff person and contain all of the required documentation.

The staff records did not contain date of hire, emergency contacts, proof of ID, 2 references, qualifications, and orientation documentation.
