An announced, on-site initial inspection was initiated and completed on 10/14/2025, as a part of the conditional licensure process.. The on-site inspection began at 1p.m. and ended at 2:35p.m. The inspector reviewed compliance in the areas listed above. There were no children present and 1 caregiver; the point total was zero. The inspector reviewed no children?s records and 1 caregiver and 2 household members records on-site. This inspection included: document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
