Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 12/11/2025, as a part of the Conditional licensure period. The on-site inspection began at 12p.m. and ended at 12:30p.m. The inspector reviewed compliance in the areas listed above. There were no children present and 1 caregiver. The inspector reviewed no children?s records and no caregiver or household member's records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
